Acceptance
In ticking 'Yes' I agree that:

if my application is accepted, I will be employed as a casual worker under the Horticulture Award 2010;
I must take at least a 15 minute break in the morning and afternoon and a 30 minute break for lunch and I will not be paid extra if I do not take these breaks;
my application will not be considered if the forms below are not completed and emailed to [email protected] within 24 hours of lodging this form;
submitting this form does not guarantee any work; and
I am fit and healthy and do not suffer from any condition that will prevent me from undertaking fast-paced physical activity including working at heights.
